MVP will be mailing individual, personalized applications to your home address (the most recent address you provided to CSI) next week. You have until February 14, 2008 to sign and return the application to MVP.  Rates for the plans provided in the application.  If you have any questions regarding the application or certificate of insurance coverage, contact information will be provided with the letter for easy reference.
